PVOLVE's E-sign Disclosure

Pursuant to the Federal Electronic Signatures in Global and National Commerce Act (“E-Sign Act”) and before providing electronic signatures, please review PVOLVE's E-sign Disclosure below.  

By clicking on the “I Agree” (or similar) in relevant contracts, policies, waivers and/or releases made available to you electronically (“PVOLVE Forms”), you hereby adopt the action described as an indication of consent and your electronic signature. You agree that the use of electronic signatures, such as your act of clicking, checking a box, or otherwise manifesting your assent constitutes your signature (hereafter referred to as "E-Signature"). Furthermore, you agree that your E-Signature is an acceptance of and agreement to PVOLVE Forms as if they were signed by you in writing, and that your E-Signature is the legal equivalent of your manual signature. You also agree that no certification authority or other third-party verification is necessary to validate your E-Signature and that the lack of such certification or third-party verification will in no way affect the enforceability of your E-Signature or any resulting contract.
